Mission Statement
PrimeCare Community Health, Inc. (PrimeCare) is a Federally Qualified Health Center (FQHC) serving under-resourced, west-side Chicago communities since 1992. We offer primary care, prenatal and delivery, dental, behavioral health, psychiatry, medication-assisted treatment (MAT), dermatology, case and care management, and eligibility and assistance services at six community-based facilities. Our mission is to promote wellness and provide accessible, compassionate, and culturally respectful care.
